Schedule Change Request

Please complete this form by August 18th. No scheduling request forms will be accepted after 8/18/25. Administration will make every effort to accommodate your request prior to the beginning of the school year. However, if you have numerous requests on your schedule, the counselor may wait to meet with you in person to discuss options during the first week of school. No request will be changed based on a teacher preference or lunch periods. All requests must be made in writing (with parent signature). You may drop off the signed form in the main office or scan the completed form to your counselor Ms. Bash-Wilson ([email protected]) or Ms. Nagy ([email protected]).
