New Student Registration

Thank you for contacting our school regarding enrollment for your child(ren).  Please download and complete the forms listed below (downloadable forms)  If you need a paper copy, please contact our office.   Please return completed forms to our office as soon as possible by email, fax or in person. Contact information is listed at the bottom of the page.

In addition to these forms, we will need:

  • Student Accident Insurance
