New Student Registration

Thank you for contacting our school regarding enrollment for your child(ren).  Registration forms are listed at the bottom of the page (downloadable forms).  Please complete and return all forms to our office.  If you need a paper copy, or have any questions, please contact the office at any of the numbers listed below.  

In addition, we will also need the following: 

• • • Birth Certificate
    • Proof of Residency (Driver’s license, Utility Bill)
    • Current Immunizations
    • Any custody/guardianship paperwork

The following forms will need to be completed in the skyward portal after registration is complete:

  • Acetaminophen Letter
  • ASD Technology Form
  • Chromebook Insurance, $24 annually
  • District Bus Regulations
  • Handbook Acknowledgement
  • Student Accident Insurance 
  • Do Not Photograph (opt out)
